In accordance with it, the procedure for identifying persons in need of outside care, assessing their living conditions, including elderly people in need of outside care and persons with disabilities in the register of single and lonely residents, forming an individual plan for the provision of social services and assistance, as well as monitoring the effectiveness of social services and assistance provided.

A social worker of the Inson social service centers for mahallas, a patronage nurse and the chairman of the citizens' self-government body are constantly working to identify persons in need of outside care, and to enter these persons into the Register, fill out a questionnaire at the Inson center.

Persons in need of outside care are identified as follows:

• in the performance of official duties of a social worker of the mahalla, a patronage nurse and the chairman of the citizens' self-government body, including on the basis of a questionnaire formed as a result of an apartment-by-apartment inspection;
• on the basis of written, oral or electronic appeals from elderly persons, persons with disabilities or their legal representatives or other individuals and legal entities sent to the IP "Social Protection".
